Community Navigator / Reaching Home Coordinator

The Community Navigator provides outreach and support to individuals experiencing homelessness or those at risk of homelessness. This role advocates for people facing addictions and crisis situations, connecting them with treatment programs, housing supports, and community services.

What We Do

The Community Navigator works one-on-one with clients and families to:

• Assist in finding safe and stable housing
• Provide advocacy and support when dealing with agencies or bureaucracies
• Connect clients with addictions treatment and recovery programs
• Link clients to local social support services
• Support families of individuals experiencing homelessness in accessing needed resources
